I know there was a time in history (maybe around the time of your mother's birth?) where doctors gave women shots in their breasts in order to prevent milk production so they would be forced to use formula. Sometimes the women wouldn't even be told this was being done to them. So if this note says the breasts may be tender and have discharge, and doesn't mention how to breastfeed, my suspicion is that's why.
